Executive SummaryThis document is the final report of the Data Acquisition and Analysis Working Group (DAAWG) of the Flight Safety Foundation (FSF) Approach-and-landing Accident Reduction (ALAR) Task Force (Appendix D contains the complete listing of participants). The DAAWG was established in August 1997 to independently analyze data that may lead to the identification and/or resolution of approach-and-landing safety issues. Activities pursued by the DAAWG included: high-level analyses of 287 fatal accidents; detailed case studies of 76 accidents and serious incidents; and the assessment of key crew behavioral markers isolated in the occurrences and in the line audits of about 3,300 flights. The DAAWG is also conducting an economic analysis of the cost of approach-andlanding accidents (ALAs) to the industry (in progress)
