Summary:
This video demonstrates the use of prolonged exposure therapy (PE) with a Black, adult man presenting with posttraumatic stress disorder (PTSD) following a motor vehicle accident. In the opening interview, Dr. Peter W. Tuerk facilitates a conversation with Dr. Sheila A. M. Rauch about PE. They introduce PE as an effective treatment for PTSD. Dr. Rauch demonstrates PE in the third session with the client. In this session, the client engages in imaginal exposure of his motor vehicle accident for the first time. The client has been experiencing increased nightmares and intrusive symptoms of his motor vehicle accident in light of teaching his son how to drive. Dr. Rauch provides psychoeducation on how to approach the trauma memory through imaginal exposure and uses metaphor to connect with the client. Dr. Rauch then supports the client as he approaches his trauma memory through validation, and probes the client to explore the fear and panic he felt during the accident. During the imaginal exposure, Dr. Rauch monitors the client's distress through use of the Subjective Units of Distress Scale (SUDS). Dr. Rauch and the client then process the emotions of the trauma memory. At the conclusion of the session, Dr. Rauch and the client collaboratively set homework (e.g., imaginal exposures, in-vivo exposures). Following the therapy demonstration, Dr. Tuerk and Dr. Rauch identify and discuss core strategies and skills of PE, using examples from the session. The demonstration in this video features a client portrayed by an actor on the basis of a composite of cases
