- In this first session, Dr. Jennifer L. Murphy establishes rapport and prepares a client living with chronic back pain for treatment. This integrative behavioral approach draws upon cognitive-behavioral therapy, acceptance and commitment therapy, and mindfulness-based stress reduction, and emphasizes treating the whole-person. The client presents as a White man in his 60s. The client struggles to manage his pain, which has prevented him from being active and has negatively affected his relationships. The client's avoidance of situations that may induce pain combined with side effects of his medication inhibit his ability to engage in satisfying life activities. Murphy first explores the client's experience of chronic pain, how it impacts his daily life and relationships, and how he has attempted to manage his pain in the past. Then, Murphy provides psychoeducation to the client about chronic pain and approach avoidance behaviors, and discusses the evidence and rationale for managing pain with behavioral strategies. Furthermore, Murphy provides a biopsychosocial conceptualization of chronic pain to help the client reframe his understanding of what it means to live with and manage chronic pain. Together, they discuss reasonable expectations for treatment and possible goals the client may consider. In conclusion, Murphy discusses next steps for starting treatment
