Summary:
Dr. Samantha Rafie works with an adult White female client who is working to taper her use of opioids to manage her chronic back pain. The client shares that she struggles to be engaged in her children's lives without being overactive, which would lead to an increase in her pain symptoms. The client also says that her symptoms have impacted her relationship with her husband. The client adds that she, at times, feels "useless" and her negative affect influences her pain symptoms, making it challenging to reduce her opioid use. Rafie approaches the client's pain management from an integrative behavioral treatment strategies perspective. First, Rafie supportively provides psychoeducation about the relationship between pain, mood, and cognition. Then Rafie explores the client's ambivalence about reducing opioid use and helps the client envision the changes she would like to make. Rafie introduces and practices diaphragmatic breathing with the client, a technique the client can use to regulate her stress and manage her pain. The client, excited and hopeful, asks about replacing her medication with breathing exercises, and Rafie discusses integrating behavioral strategies with the client's existing medication management plan. Then Rafie collaboratively sets goals and expectations with client. The client chooses a specific goal for the next week of registering at a gym she would like to join. At the end of the session, Rafie discusses what treatment will look like going forward, and highlights the client's excitement and open mindedness about treatment
