"In this session, Dr. Anne C. Dobmeyer demonstrates the Primary Care Behavioral Health (PCBH) model of integrated service delivery with a female client with Type 2 Diabetes. Dr. Dobmeyer first introduces the integrative approach between health care providers and reviews confidentiality. She and the client then discuss the client's general medical record and history, including physical pain, tobacco/alcohol use, and other medical problems. Subsequently, Dr. Dobmeyer and the client discuss disease management and the client's past and present habits related to exercise and dieting, as well as current motivation and confidence levels and perceived barriers to healthy behaviors. In the second half of the session, Dr. Dobmeyer discusses recommendations with the client and they make plans regarding future disease management. Throughout the session, Dr. Dobmeyer discusses collaboration between various health care providers, including herself, the client's primary physician, and the client's dietician"
