- Counseling psychologist Dr. Kathleen J. Bieschke conducts a psychotherapy session with a young adult gay male African American client experiencing major depression. Dr. Bieschke works from a supportive, affirmative approach that is culturally sensitive to diversity issues as well as the depressive concerns presented by this client. In the session, the therapist seeks to ascertain the client's level of comfort with his identity as a gay man. In response to the client's initial inhibited manner, Dr. Bieschke addresses the differences in age and race between herself and the client while noting their ultimate commonalities, which frees the client to speak more openly. The client describes an early experience of sexual abuse at 6 years of age, which he perceives as playing a pivotal role in the development of his sexual preference. In encouraging the client's expression of his sense of self, Dr. Bieschke observes the client's conflictual, negative feelings about himself and difficulty with acceptance of his gay male identity. She empathizes with the inconsistencies in support provided by the client's caretakers, noting the ways in which shifts in care related to challenges associated with his sexual identity and sense of self. Alongside her attention to the client's history, the therapist assesses the client's current support system and depressive symptoms and risk for suicide. She encourages opportunities for pursuing meaningful goals and developing more fulfilling relationships
