- The unnumbered leaves of the second half contain indices including the index to the History of the Exchequer.
- Printed by William Bowyer. Bowyer's records show 500 copies printed on crown, 44 on demy, and 6 on royal. The large-paper copies may show variation in placement of press figures, or in their presence or absence on a given leaf; the details are unclear. Reissues in 1741, with new titles also printed by Bowyer, consist of either the complete work, with a cancel title page; or 'A compleat index to Mr. Madox's History of the Exchequer', with a new title page (see T97065, T97066, and T97068).
