- A folded sheet with 8 panels on each side when unfolded. The "outside" contains a title panel, a panel headed "Trustee's Certificate", two blank panels, and four panels containing the decorated versos of two coupon notes each. The "inside" contains the terms of the bond spread over four panels and four panels containing two coupon notes each. All panels are unnumbered.
- The "Trustee's Certificate" contains a blank space for the signature of the secretary of the Utah Colonization Fund, Inc.
- The terms of the bond contain blank spaces for the number of the bond, the name of the bearer, the day and month of issue, the signatures of the president and secretary of the Jewish Agricultural and Colonial Association, and the seal of the same organization.
- Each coupon contains a blank space for the number of the bond.

- CJS Archives copy is number 1494 (the number supplied by a red ink stamp on title panel, at head of terms of the bond, and on each coupon), purchased by Jacob H. Schiff (the name supplied in manuscript in the terms of the bond) on 14 April 1913 (the date supplied in manuscript, with printed "1912" corrected to "1913").
