Summary:

Administrative health data are data continuously collected by health maintenance organizations and payers of health services. It is a valuable source of information for observational studies. The use of these data for research projects is also relatively low cost. This case study provides an example of utilizing this type of data in an epidemiological research project. The project was a retrospective cohort study that followed adults in Alberta (a province in western Canada) from 2008 to 2018 and developed aspiration pneumonia (AP). One of the objectives was to estimate the incidence rate of AP in Alberta. This case study focuses on the development of a method to identify patients with AP in the administrative data for incidence calculations. Accurate identification of patients is a priority because it minimizes misclassification bias and positively impacts the internal validity of research findings. Practical examples are included to demonstrate several key concepts, including algorithm sensitivity and specificity. Some tools that are useful for students to utilize administrative data in their research are also provided.
