"Medication errors include failure to properly identify the patient, heavy workloads, staff shortages, noise and distractions, fatigue, sound alike drugs, misuse of abbreviations, an illegible prescriptions, inappropriate dosage and infusion rates, incorrect time, and poor communication during change of shift.
