Sarah Swords describes the demographics of the rapidly growing older adult population and focuses on geriatric care management - an exciting and effective way to address the complex needs people face as they age. She outlines the processes of engagement, assessment, and intervention planning using a biopsychosocial perspective. These processes come alive during Swords' initial geriatric care management session with Julie, a 64 year old woman whose husband recently retired. Julie had been the primary caregiver for her mother until her recent death and now she reports concerns about her own adjustment and health. Swords builds rapport and begins the assessment process of the multiple transitions and stresses that Julie currently faces.
