- Summary:
- These are leaves surviving from a manuscript of the Hebrew translation of Avicenna's Qānūn fī al-ṭibb (translated by Nathan ben Eliezer ha-Me'ati of Cento); surviving in the text is עונות טובות (from Book 4, ch. 53; f. 1v) מאמר העילופיית הלחיית (from Book 4, ch. 61; f. 2r). Item worn, with considerable loss to text; item probably survived inside a binding.

- Layout: Written in 31 lines (surviving column length from f. 1v); textblock ruled in drypoint; titles aligned into columns.
- Script: Written in a Sephardic-Iberian semicursive script; titles written in a Sephardic square script.
- Origin: Written in Spain or possibly North Africa, probably during the second half of the 14th century.
- Corrections in a 16th century Sephardic cursive hand (margins and inside lines).
