Summary:
This is a manuscript roll from the Tefillin (also known as phylacteries), specifically the Tefillin placed on the arm (Tefillin are in a set; one is placed onto the forehead and the other on the left bicep), as the scroll contains all Biblical pericopes consecutively. Rashi's formation follows the order written in the Bible: קדש לי כל בכור (Exodus XIII:1-10) and the successive והיה כי יביאך (XIII:11-17), then שמע ישראל (Deuteronomy V:4-8) and והיה אם שמוע (XI:13-22); see (among other sources for the Tefillin of Rashi) Maḥazor Vitry 512 and Sefer ha-Terumah 206.

Layout: Written in four columns of seven lines; blind ruling (with pricking on both ends) following the Ashkenazic custom (Isserles to Shulḥan ʻarukh Oraḥ ḥayim 32:6), and each passage is written in an individual column (ibid 32:36).
Origin: Written in Germany (or Bohemia/Moravia), circa 18th century.
Script: Written in an Ashkenazic square script; the ע' and ד' written in bold (third column); written with Tagin (letter crowns on specific letters, using both single and triple crowns).
Binding: Parchment clasp, possibly originally a piece of the original roll (saved with the roll, possibly because of עיבוד לשמה, or intentional preparation for phylacteries).
