- Imperfect: many pages missing, some mutilated with initials cut out.
- BCP colophon with printer's device (Mck. 125 [alpha]): Imprinted at London by Newegate Market next vnto Christes Church, by Richarde Iugge, printer to [sic] Queenes Maiestie.
- Signatures: BCP a¹² B⁸ A⁶ B-P⁸ Q⁶ ; Psalter A-I⁸ K⁶ A²-Q⁶ R⁴.
