Biography/History:
A prominent physician with a large practice in Euclid, Ohio. Hazen was one of twelve children born to Azel and Lovisa Fletcher Hazen in North Hero, Vermont. When Hazen was a young boy the family moved to Euclid, Ohio. Around 1825 Hazen returned to North Hero for a few years where he met and married his first wife Huldah S. Dodds and they had six children. Huldah died in 1843. Two years after later Samuel married Clarissa Converse on June 5, 1845. The couple had seven children.
Summary:
This volume is a physician's account book kept by Dr. Hazen in Euclid, Ohio spanning the years from 1852 to 1862. The volume consists of patients' names, services performed, and cost of services provided. Hazen was a family doctor and saw adults and children. Many of the services consisted of visiting patients and providing medicines. The ailments of each patient are not mentioned with the exception of tooth extractions. In some cases the medicines provided are listed such as balsams, cordials, and plasters. Noted in the account book are payments received from patients. Some Jewish families of Euclid are listed as patients. Written on the inside front cover: Dr. Samuel Hazen. Some notes and math calculations made on inside front and back covers. Covers and spine are detached with many loose leaves and gatherings.
