- Provides background on problem of medical errors by health care providers and analyzes medical error reporting systems. Discusses legal and policy issues facing State mandatory reporting systems and major national voluntary reporting systems, and identifies design features of effective reporting programs. Describes and compares provisions in H.R. 663 and S. 544, both the Patient Safety and Quality Improvement Act, to establish legal protections for data and reports on medical errors to encourage voluntary reporting.
