Return "of (a) the number of companies to which the registrar has sent applications for their annual return in consequence of default during each of the last six years; (b) the number of such cases referred by the registrar to the Board of Trade for prosecution in each of these years; (c) the number of such cases in which penalties were imposed; and (d) the amount of the penalties recovered."
