The only known copy, held by the American Antiquarian Society, is filled out in manuscript appointing Clark Chandler captain of the Idle Society in Worcester, Mass. Dated the first of November 1799, and signed by Oliver Vose, secretary.
Probably printed by Charles Prentiss, the only printer active at Leominster in Nov. 1799.
Printed area measures 18.2 x 19.3 cm.
