- Yarnall Library of Theology (Saint Clement's Church, Philadelphia, Pa.) copy; on permanent deposit at the University of Pennsylvania Library. Probably acquired by the Yarnall Library of Theology from Voynich. With the bookplate of the Yarnall Library, St. Clement's Church, on front pastedown.
- Rubrication: Usually only the beginning of each book rubricated. Initials supplied in red, blue or green; paragraph marks in blue or red; capital strokes in red.
- Illegible early ms. inscription on t.p.; a few other ms. notes in margins throughout. Vellum finding tabs, some with early ms., at the beginning of each book.
- Bound in contemporary blind-tooled leather over wooden boards with vellum backed spine; 4 raised bands on spine; 2 leather-and-metal clasps (intact) with metal catchplates on front cover; author and title written on a paper label pasted to spine. Corners of top board damaged; numerous wormholes in both boards.
- Wormholes throughout, especially in final quires, with some minor loss of text. Occasional dampstaining at top margin.
