Summary:
Chronic disease affects more than 100 million Americans and represents half of total health care costs. Unfortunately, because of the continued dominance of a narrow medical model, educational or skills training in patient self-management of chronic illness is still viewed as merely supportive, as opposed to being a major therapeutic element in patient care. Considerable evidence shows that most of the population is not receiving the self-management preparation they need, even in areas where the evidence base for it is strong. Patient Self-Management of Chronic Disease draws attention to the necessity for self-management preparation for patients and families and how it can be accomplished. It describes evidence-based best practices for patient/family self-management preparation for a wide variety of diseases, illnesses, and symptoms.
