Fair condition. Leaves very brown. The manuscript is torn accross the lower left corner. Ff. 4 and 8 are colored yellow. Text has no dandas. Mistakes written over or indicated by short vertical strokes. One lengthy marginal addition in another hand.

Begins, f. 1v: o ̐ācamya. pr̄aṇān āyamya. deśakālau smṛtvā. vrāhme mūhurtte cotthāya scaśīrṣe sahasradalāmbujodaraṁ karṇikāmadhye śrīgurupādukāṁ dhyātvā (verse 1). ādhāranābhihṛdaye bhrūmadhye śrīmadakṣararūpaṁ śivaṁ dhyātvā tatprabhā puñjapiñjarīkṛta (verse 2).

Text ends, f. 8v: mukhe saṁ namaḥ nāsāyāṁ haṁ namaḥ netrayoḥ oṁ namaḥ lalāṭe ks̄aṁ namaḥ vraṁhmarandhre.
