- Super-chancery folio, printed primarily in double columns. Leaf a1v: 47 lines to a full column, plus headline; area of text: 215 (230) x 128 mm.; col. width: 59 mm. Initial spaces with guide letters; spaces for paragraph marks. With signatures; without foliation or catchwords. Register on leaf Q10r.

- Purchased for the Penn Libraries in 1910 with support from the George L. Harrison Foundation for the Encouragement of Liberal Studies and the Advancement of Knowledge, a fund established in 1895 by a donation to the University of Pennsylvania by Charles C. Harrison, in honor of his father. The bookplate on front pastedown reads "The Geo. L. Harrison Foundation".

- Penn Libraries copy bound in full early blind tooled leather over square-edge wooden boards (mended with modern leather); spine rebacked in modern brown morocco(?) with gold-tooled black leather spine label ("LIBER GRATIAE"); 5 raised bands on spine; evidence of two fore-edge clasps and catchplates; remains of parchment finding tab affixed to fore-edge of leaf P2r.
