Broadside (chancery half-sheet, oblong format): 23 lines, including 2 headlines; area of text: 128 x 229 mm. Headlines read "Forma confessionalis." (line 1) and "Forma absolutionis." (line 15). At the end of line 2 and the beginning of line 3, space has been left for the names of the recipients. In line 12, space has been left for the name of the Franciscan subcommissary; at the end of line 14, space has been left to complete the date of issue. Rubrication not called for.
