- Penn Libraries copy: lower portion of title leaf excised and replaced with a blank leaf without loss of text; head outer corners of leaves m6-m7 each excised with some loss of text and replaced with a blank leaf upon which the missing text (and, in the case of leaf m7v, a missing woodcut initial) has been supplied in ms.
