- Chancery quarto, printed in double columns. Leaf [2]r: 36 lines to a full column; area of text: 143 x 95 mm.; col. width: 42 mm. Initial spaces with guide letters. Section marks and headings printed in red. Without foliation, signatures or catchwords.

- Rubrication: initial and capital strokes supplied in red on leaf [2]r only.
- Provenance: From the library of Henry Charles Lea. With his bookplate affixed to back pastedown and his autograph (1892) on recto of blank leaf [1]r. This leaf also has 3 early ms. former ownership inscriptions (1 of Augustinian monastery of St. John the Baptist in Rebdorf, Germany, 1 partially illegible and 1 effaced) and early ms. contents note on upper portion; ms. notes in pencil in Lea's hand on lower portion.
- Bound in one-quarter blind-stamped pig over wooden boards; 3 raised bands on spine; ms. title in ink and shelf marks in ink and pencil on spine; evidence of clasp and catchplate; vellum leaves with early ms. (including part of the antiphon "In prole mater, in partu virgo" with musical notation, and texts related to the feast of the Exaltation of the Holy Cross) in brown and red ink used as binder's waste.
- Leaves [2]-[112] numbered "1"-"111" in ms. in upper margin; some ms. underlines, marginal marks and notes in ink or pencil throughout text.
- Leaf 95 torn and mended across upper portion. Many pinpoint wormholes through boards; pastedowns and first few leaves lightly wormed with occasional damage to text; occasional wormholes throughout bookblock.
