Chancery folio. Leaf a4r: 37 lines; area of text: 208 x 105 mm. Initial spaces, most with guide letters; spaces for paragraph marks. With signatures; without foliation or catchwords. Register on leaf x5r.
Signatures: a-c¹⁰ d-k⁸ l⁶ m⁸ n⁶ o⁸ p¹⁰ P⁶ pp⁴ q¹⁰(-q9,q10) r¹⁰ s-t⁸ u-x⁶.
Leaves a1 and x6 are blanks. Leaf pp1 is unsigned.
On leaf q2r, the "q" of the signature mark is printed upside-down. A variant: "q" of the signature mark printed in the correct orientation. Cf. BSB-Ink exemplar 1 (2 Inc.c.a. 1020).
GW and Bodleian each give signature statement: a-c¹⁰ d-k⁸ l⁶ m⁸ n⁶ o⁸ p¹⁰ P⁶ pp⁶ q⁶ r¹⁰ s-t⁸ u-x⁶. (BSB-Ink gives no signature statement but exemplar 1 (2 Inc.c.a. 1020) has 2 stubs bound in between last leaf of gathering q and first leaf of gathering r.)

Penn Libraries copy bound in early full blind-tooled parchment over square-edge wooden boards; evidence of 2 leather and metal clasps and 2 catchplates; 3 raised bands on spine; 2 portions of an early parchment leaf from a liturgical music ms., probably a gradual, with texts and music for the feasts of St. John the Baptist (June 24) and Sts. John and Paul, martyrs (June 26), ruled in dark ink (first line below top line) and written in a Gothic bookhand in dark ink with rubrics and initial in red ink and neumes in dark ink, used as binder's waste; printed paper slip ("Printed in Germany") affixed to inside left board.
Penn Libraries copy: bifolia q4.7 and q5.6 bound in out of order in each other's places (i.e. leaves bound in the following order: q5, q4, q7, q6).
Penn Libraries copy: boards, spine and some leaves at beginning and end of volume lightly wormed; leaves q1-x6 stained in fore-edge margin.
