Evans supplies imprint: [Cambridge: Printed by Samuel Green. 1689]. However, this item was included on a printers' bill presented to the General Court by Bartholomew Green, who, in 1689, was printing for his brother Samuel in Boston, rather than for the elder Samuel Green at Cambridge.
